Scientists have found that neuroangiostrongyliasis combination therapy using albendazole and doxycycline offers superior neuroprotection. Angiostrongylus cantonensis causes eosinophilic meningitis, a condition often seen in parts of Southern India. Although clinicians use albendazole to kill larvae, the drug often worsens inflammation via antigen release. Therefore, researchers investigated whether doxycycline could mitigate this effect through its anti-inflammatory properties.
The study demonstrated that early combination therapy effectively eliminates parasites. Moreover, it reduces perivascular inflammation and hemorrhagic changes. Consequently, the dual approach provides a more stable recovery than monotherapy.
Using high-resolution MRI, the team observed a marked decrease in brain hyperintensity. This finding indicates that the treatment prevents extensive cerebral edema. Additionally, biochemical analysis showed a restoration of the IL-33/GFAP balance. Because these markers signify astrocyte and microglial activation, their normalization confirms reduced neuroinflammation.
Furthermore, serological tests showed lower immunoglobulin levels in the treated groups. This suggests that the combination therapy modulates the systemic immune response. Therefore, these findings support the use of doxycycline as a neuroprotective adjunct in parasitic meningitis. Notably, the treatment window is critical, as early intervention yielded the most consistent improvements. Similarly, the study highlights the importance of glial regulation in managing parasitic brain injuries. In addition, the reduction in specific IgA/G/M reactivity proves that the therapy affects multiple immune pathways. Thus, this regimen may become a future standard for managing rat lungworm disease.
Doxycycline acts as a neuroprotective agent. It attenuates glial activation and matrix metalloproteinase activity, which reduces the inflammation caused by dying parasites. This helps prevent the clinical worsening often seen with anthelmintic monotherapy.
The therapy restores the balance of interleukin (IL)-33 and glial fibrillary acidic protein (GFAP). These markers are typically elevated during astrocyte and microglial activation. Normalizing these levels indicates a reduction in brain-wide inflammatory damage.
While albendazole is a standard treatment, this specific combination is currently supported by preclinical mouse models. These findings provide a strong rationale for future clinical trials to confirm its safety and efficacy in humans suffering from neuroangiostrongyliasis.
