Waiting for clinical decompensation before titrating therapy deprives patients of substantial survival benefits. Overcoming these barriers requires continuous clinician education, standardized hospital discharge protocols, and systematic outpatient monitoring.
Modern management requires pragmatic sequencing strategies to achieve complete quadruple therapy promptly. Historically, guidelines recommended sequential titration, taking months to achieve target doses. Consequently, high-risk patients remained vulnerable during early post-discharge windows. Contemporary evidence supports simultaneous initiation of all four drug classes at low starting doses within two to four weeks. Furthermore, this approach offers immense relevance for healthcare practitioners globally, including in India. In India, heart failure strikes individuals at a younger age, commonly accompanied by diabetes and coronary artery disease. Clinicians previously faced affordability barriers; however, the widespread availability of generic dapagliflozin and affordable sacubitril/valsartan has transformed treatment feasibility. Indian physicians can leverage these cost reductions to establish quadruple therapy early during hospitalization. In addition, strengthening primary care coordination ensures vigilant electrolyte monitoring and prevents premature drug discontinuation. Ultimately, proactive implementation of guideline recommendations will bridge the translational divide, ensuring life-saving therapies reach heart failure patients worldwide.
