Selecting once-daily single-tablet antiretroviral regimens with high genetic barriers to resistance also provides forgiveness against occasional missed pills. In addition, clinics must offer rapid, accessible screening for syphilis and other sexually transmitted infections. Ultimately, combining pharmacotherapy, behavioral counseling, and sexual healthcare establishes a durable foundation for viral suppression. Healthcare teams can thereby protect individual immune health while curbing viral transmission.

Clinicians can safely prescribe oral naltrexone, extended-release injectable naltrexone, or acamprosate to reduce alcohol cravings and heavy drinking days in individuals living with HIV. These evidence-based pharmacotherapies do not exhibit adverse pharmacokinetic interactions with modern antiretroviral regimens. However, physicians should monitor hepatic transaminases regularly, particularly in patients with pre-existing viral hepatitis coinfection. When combined with supportive behavioral therapy, these medications improve alcohol abstinence, thereby promoting consistent medication adherence and long-term virological suppression.
